- Signed Carrier Dispatcher Agreement (Power of Attorney)
- Copy of Auto-Liability & Cargo Coverage
- Copy of Carrier’s Operating Authority (MC Permit)
- Carrier Setup Package & References
- Copy of CDL (For our fleet customers need each driver being dispatched )
- Certificate of Insurance, Naming UB Dispatch as Certificate Holder
- Signed W-9
